Per the Massachusetts COVID-19 state guidelines, as of May 29, 2021 masks remain mandatory in healthcare facilities. ALL INDIVIDUALS ENTERING OUR OFFICE ARE REQUIRED TO WEAR A MASK. Masks must cover your mouth and nose and be worn at all times until your clinician informs you otherwise.

If you can answer “YES” to any of the following questions, you do need to call the office prior to your appointment.

In the past 14 days:

• Have you tested positive or been exposed to COVID-19?
• Have you experienced any symptoms related to COVID-19?
• Have you traveled out of the country?

If you have an upcoming appointment:

¨   All patients must call 781-329-1234 to check-in remotely upon arrival.

¨   If you have any changes to your address, phone numbers, email, or insurance(s), please call the office to update PRIOR to your appointment.
